Thomas L. Lim, O.D. VSP Eye Doctor - San Jose
A graduate of the Illinois College of Optometry, Dr. Lim is an active member of The American Optometric Association, California Optometric Association, Santa Clara County Optometric Society, Banzai Eyecare Group and the Cleinman Performance Network. Dr. Lim was a recipient of the Young Optometrist of the year award in Santa Clara County and California, as well as the Optometrist of the year in Santa Clara County. He has also served on the San Jose Mercury News Reader Advisory Board.
Being very involved in the optometry profession, he has served as the Political Activities Coordinator for the Santa Clara County Optometric Society for over twenty years and is on the board of trustees of the California Optometric Political Action Committee and the California Optometric Laser Associates. He also serves as the Northern California Political Activities Coordinator for the American Optometric Association.
Dr. Lim was awarded the national American Optometric Association (AOA) Political Action Committee Representative of the Year Award in 2004 and 2005 at their Congressional Conference in Washington, DC. He was selected because of his commitment to the profession, both locally as well as nationally.
Dr. Lim is well respected in the profession. He is one of the eye doctors with the San Jose Sharks, providing eyecare during their annual physical exams in the beginning of the season. He also serves on the National Hoya Vision Care Eyecare Practitioners Advisory Panel, recently partipated in a panel discussion with other eye doctors on wavefront technology and how it fits in today's eye exam and the iZon High Definition Lens at the International Vision Expo West in Las Vegas (the largest meeting of optometrist in the country) and lecturing to local eye doctors on the effects of eye fatigue while using the computer and how it is treated. Dr Lim was interviewed by radio personality Pat Farnack of radio station WCBS 880 in New York City and Rob Artigo of radio station KGO in San Francisco on high definition vision and the high definition iZon lens and how it can increase sharpness of vision, clarity and depth perception.
He has been married for over thirty years and has two adult children. Dr. Lim is an active member of the First Chinese Baptist Church in San Francisco.
Dr. Hung D. Mac VSP Optometrist - San Jose
Dr. Hung D. Mac attended San Jose State University, where he graduated with a Bachelor of Art degree in Biochemistry in 2000. In 2004, he earned his Doctor of Optometry degree from the Southern California College of Optometry. He received Cum Laude honors at both insitutions. Dr. Mac then completed a prestigious Residency in Cornea and Contact Lens at the Southern California College of Optometry, which involved specialty contact lens fitting and management of complex cases, including keratoconus, post-surgical corneas, corneal refractive therapy (CRT), pediatrics and ocular prosthetics. During his residency, Dr. Mac thoroughly enjoyed serving as a clinical instructor to the optometric interns. His clinical experience, as well as the patients he cared for, has inspired him to continue corneal management and specialty contact lens fitting.
During his free time, Dr. Mac enjoys traveling and spending time with family. His internships and medical mission trips have brought him to El Salvador, Salt Lake City and into a rural Navajo Indian Reservation in Arizona. He plans to continue participating in medical mission trips to South Africa and his birth country, Vietnam.
Dr. Mac is a member of the California Optometric Association, American Optometric Association, Santa Clara County Optometric Society, and Beta Sigma Kappa Honors Fraternity.

InsuranceRoutine Vision Coverage
Some individuals are fortunate enough to have “Routine Vision Insurance” through their employers. Some plans pay toward the optometric eye exam only, others will sometimes pay towards the contact fitting fee and some even pay toward materials. If you are uncertain about your benefits, you will need to check 1st with your human resource department or call your insurance company direct. Simply ask: Does my plan provide optometric vision benefits? Can I see any eye doctor of my choice? What does my plan allow?
If you are allowed to see any eye doctor of your choice we will verify your coverage, file the claim electronically and accept assignment of benefits for you. To verify your coverage, we will need a phone number for verification of benefits, a group, plan or policy number, the social security number of the insured, member and patient’s date of birth, and name of employer. Verification of benefits must be verified 24 hours prior to your appointment. When you arrive for your appointment we will need a copy of your insurance card. You will be expected to pay in full the difference in the amount allowed at time of service. If for any reason the insurance company pays less than quoted you will be responsible for any amount unpaid.
